He reached under his jacket into the left breast pocket of his Oxford shirt, with his right hand. He then brought both of his hands together. He brought one of his hands to his mouth (I think it was the left hand), and put something in his mouth...very stealthly.
I thought he must be putting a lozenge in his mouth. But then he kept his mouth very still. (My thought was - he 's holding his mouth oddly still.) And then he continued on with nothing in his mouth.
He did not take a drink of water at the same time, but many can swallow pills without water.
The only other explanation I can think of is that he decided to swallow the lozenge instead of sucking it like one normally does.
Note: I will try and find the hearing online, or on the CSpan website, to look at the hearing again, and see if I can confirm what I saw.
I will also note that several of us (on DU) have noticed him be very "twitchy" as things proceed, particularly when pressured by some Dem Senators.
P.S. He is a sly one - he sometimes tried to distort the question being asked of him, so he can obfuscate. Slightly unhinged and sly - a bad combination.

But a nitro pill for his heart would be a reasonable thing. I guess I am suspicious of him because of how he presents overall...and his past...and the letter Caroline K.S. wrote.

He was not doing a good job of covering up his hypocrisy and flip flopping. And he was playing gaslighting games with pretending he misconstrued the question. Can you say obfuscate?
