My genius daughter has misplaced her Covid vaccination card, she thinks it probably got thrown out a couple of weeks ago. She got her shots at a Publix pharmacy so I told her to head over there and see about getting a replacement card, but in searching online it looks like they may not be able to issue a replacement.
She has something coming up in November which will require proof of vaccination. If she can't get a replacement card the only other thing I can think of is to take both shots all over again (or just get the single shot J&J), but that raises a HUGE red flag about safety. I haven't been able to find anything on that, I'm wondering if anyone else out there has seen anything.

I would def not retake as we have no idea if that would be safe!

(5,236 posts)The nurse who vaccinated me suggested that I take a photo of my card, not that that helps your daughter now.

(106,341 posts)that is searchable through the Dept. of Health online(e.g., Colorado) or if not searchable, allows you to make an email inquiry to the program staff to check for you.
But, PublixPharmacy can definitely provide you proof of vaccines. Whoever said they couldn't is not informed. Under HIPAA they can't give the info to you, but to the patient? Absolutely.

(106,341 posts)Whether or not they give her a new CDC card, they CAN and MUST print out her vaccine information.
Again, though, they are supposed to be reporting to the state health dept. registry. So, that is your alternative.

(28,361 posts)I suspect that the injection vial label gets affixed to the info. I assume it probably gets into some database.
I took a photo of mine, in case I lose the paper card.

(14,663 posts)Her insurance company will have records. They cant replace her card but they can tell her the date of the vaccination, the manufacturer, and the amount they paid to the pharmacy.
